I have a resident who came in with rt hemicranial defect s/p placement of synthetic cranioplasty. History of right hemicraniectomy for osteomyelitis. History hemicraniectomy for hemorrhagic infarction. History of evacuation of extra axial hematoma. History of replacement of right hemicranial bone flap. History of large right hemispheric infarct.

I'm not sure how to code this. The only code I got so far is V12.54 for the infarctions. Help, please. Thank you.
