I know someone who is bundling and E/M service and vascular ultrasounds under 97499 for a certain carrier. There is an appropriate E/M code and the following diagnostic CPT codes that are available 93880,93931,93888. I have looked in the CPT manual to see if it clearly states that you have to document the more appropriate codes but cannot find that information. Any help would be appreciated
