Hello,
I am hoping to get some clarity on a coding scenario I came across. The provider documented Anemia of Chronic disease, ESRD and Hypertension all separately in the progress note. So based on coding guidelines, it states a causal relationship between the HTN and ESRD and also the Anemia and the CKD. Should both be coded together I12.0, N18.6 and D63.1? The provider didn't link any of them together in the note.
 
Yes, I am not seeing an edit for these codes together. I would run it through your company's encoder to be sure, but otherwise it should be okay to code that as there is a casual relationship between all Dx listed here.
