the provider documented in the record that pt has multiple medical conditions and list all of them; my question is do I have to code all of them? is there a dx for multiple medical conditions? or just code the reason for the visit? Please help!!!!:confused:
 
Does the doctor document any sort of treatment or management for the list of conditions that he/she names in the the encounter? Or is it just a laundry list of DXs? If the condition is documented with treatment/management, yes, you code them accompanied with the correct cpt code. If there is no management listed, you refrain from coding it.
