If the doctor only states in the documentation placenta previa, should placenta previa with hemorrhage be coded?
 
If not specified you code as w/ hemorrhage.

Looking at the descriptions in the ICD book

O44.0X says "specified as w/o hemorrhage" which means to me they must specify no hemorrhage in the notes

O44.1X says NOS or With hemorrhage, so Id use it based off the NOS as its not specified. You should probably let the doctor know they need to be specific as to whether there is hemorrhage or not so you can be the most exact. When they set up the codes they usually choose the most common option if they don't create a separate NOS. The reason is I guess its not as common to present without hemorrhage when having the condition.
