A doctor performed a laparoscopic assisted vaginal hysterectomy attempted, total abdominal hysterectomy, salpingo-oophorectomy, and bilateral salpingo-oophorectomy. Are the correct codes 58150 and 58662-51 or just a 58552?
 
i WOULD CODE THE 58150-22 and include V64.41. Be sure to send the operative note and an explanation as to why the additional should be paid.
