If I have a dx code Z85,21 and it is wanting a tobacco code Z87.89 then the physician needs to document about tobacco use correct? If a physician has a patient and he brings him back in the office let’s say 6 or 8 months later and does office visit and scope (31575). I know that Z85.21 and Z87.89 will not go though scrubber in codify it is asking for follow up examination so would I also code Z08 along with other 2 codes? Thanks
 
Instructions in Z85 Personal History has: Code first any follow-up examination after treatment of malignant neoplasm (Z08), so i believe that Z08 is the correct first diagnosis.
