patient has Degenerative joint disease of the metatarsal phalangeal

op report:
A dorsal incision was made with a sharp 15-blade. This was carried down through the skin and subcutaneous tissues. The tendon was then retracted medially and the capsule was then opened. Soft tissue debriment was then preformed. The metatarsal head was then identified.This was placed inn the wound and a 1.6mm smooth guide pin was then placed in he center portion of the metatarsal. This was then reamed with 2 removing osteophytes from the tarsal head. A second guide pin was theb placed into proximal phslsx center portion where this was then reamed with a conical reamer. The are was then irrigated.The capsule was then closed with 2-0 Vicryl. The digital nerve remained intact. The skin was then closed with 4-0 Ethibond. A sterile dressing applied.

I would code this as 28289 assuming this is on the first metatarsophalangeal joint.

Makes a dorsal incision over the 1st metatarsophalangeal joint the extensor hallucis longus tendon is retracted, and the joint capsule is entered.
