When coding for decompresson of nerves is it correct to code by the number of nerves or the incision site. If the incision is in the palm of the hand but the doctor is decompressing nerves in the digits (per his opertative) is this 64704 hand or 64702 digits?

He has me so confused. Any resource would be appreciated.
