How long can you refer back to a patient intake form for the ROS and the PFSH? Should this be updated at least once every couple years.:confused:
 
As long as it is valid and referenced by form name and date, I am not aware of any time restraints. When they are referencing these forms they are validating that they did in fact re-ask the questions but the answers did not change.

After saying that, I find it hard to believe there is no change in review of systems from visit to visit, much less year to year.

Laura is correct but the physician must make a reference to the previous form and the fact that it was reviewed with the patient. The reference as she states must be by date and location(office note, ER chart, Inpatient recored, etc)
