or

Acording to ICD-9 conventions you the coder always combine htn and ckd so it would be the 403.9x followed by the 585.x

Even if doctor doesn't write HTN can we assume it is (HBP) Hypertension.

No High BP is definitely not the same thing as HTN. Remember a coder cannot give a dx the physician has not. I see now in the original scenario it states HBP which is neither 401.9 or 403.90 I was just addressing the code combinations. I apologize for not being more thorough.
