Patient presents with Abdominal pain, constipation, and bloating. provide treats all three. so do I need to code all three? is constipation a diagnosis or a symptoms?
 
I would code all three, if treatment was directed to all three and a more definitive diagnosis wasn't given. Codes in the R section of CPT are inherently considered symptoms, and constipation is in the GI section, but it could be an isolated condition or a symptom of another condition; the provider doesn't specify.
