Good Afternoon,

I need a little assistance I am trying to code a chart for a 12-year old who was seen for vomiting with a temperature of 102.2 with tenderness in the left lower abdomen. The mother is told to take the child to ER for possible appendicitis. I know we code for the signs and symptoms but do you code for the child's actual temperature?

Did the provider use the word fever?
