If obesity was found on exam, would obesity be considered an abnormality and coded as Z00.121(Child exam w/abnormality), E66.9(Obesity,unspecified)?
 
I truely doubt that obesity is discovered during an otherwise normal exam. It will be obvious on presentation that the patient is not on the optimal height to weight ratio. So it is either a well visit for an child that is obese or it is a visit to discuss the obesity.
