Colo for history of polyps

Patient comes in for a colonoscopy who is on recall for personal history of colon polyps; I have been coding whatever CPT code is appropriate with DX code Z86.010. We have several commercial insurance carriers tell patients that it will be covered at 100% if we code it preventative. If the patient is being followed for personal history of colon polyps, can or when would they be eligible to have the routine screening colonoscopy (Z12.11) again? Would it be appropriate to use both codes Z12.11 primary & Z86.010 secondary? If anyone has guidelines that I could reference I'd be most appreciative.
