I need help coding colonoscopy via ostomy with polypectomy and colonoscopy via rectum at same sitting
 
Depends on what instrumentation was used for the polypectomy through the stoma - 44389 for cold biopsy forceps, 44392 for hot biopsy forceps, 44394 for snare....etc. Then you would also code a 45330 (flexible) or 45300 (rigid) for the rectal portion. The 'proctoscopy' portion may need a -52 because advancing to the rectal stump staple line (after a Hartmann procedure, for example) may not include any of the sigmoid and the definitions of the 45330 and 45303 both state that the scope is advanced into the sigmoid.
