I am looking for any assistance from anyone who is billing for colon polyp removal and provider is documenting in centimeters. We have been using a measurement chart that is published by the National Cancer Institute (SEER Training Modules)...See attachment.

Can anyone else provide some insight as to what is or can be referenced when provider is documenting in centimeters...if anything? I have been unsuccessful to find anything from Medicare/CMS, but have found other documentation that supports this diagram. While it is my understanding the measurements are an "average", my providers are stating that the measurements are hugely inaccurate.

Any assistance would be appreciated!
