This is a little tricky, but I was thinking with the information provided, using an unlisted code 44799, because you are performing an -oscopy procedure through the small intestine and then compare it to the colonoscopy code you were thinking. This may have to be appealed but I think the small intestine unlisted code would explain the part of the intestine that is being examined.

CPT book page 368 says to "Report pouch endoscopy codes (44385, 44386) for endoscopic examination of a patient who has undergone resection of colon with ileo-anal anastomosis (eg, J-pouch)".
