Wiki Colonoscopy with history of CUC

Patient has personal history of chronic ulcerative colitis and had surveillance colonoscopy with NO symptoms. There was NO evidence of active CUC found on the colonoscopy. Findings were polyps and diverticulosis. My question is: Is the correct primary diagnosis 556.9 or V12.70 or V76.51? I coded the primary dx as 556.9, as this is a chronic condition, however the patient is wanting me to submit the claim as preventive. I'm not sure V76.51 is the correct diagnosis for this particular case. I'd appreciate any thoughts. Thanks!
 
For our GI office, we follow the recommendations of CMS, for surveillance purposes they consider V16.0, V18.51, V12.72, V10.05, V10.06, and also chronic GI conditions, 558.2, 558.9, 555.0, 555.1, 555.9, 556.0, 556.1, 556.2, 556.3, 556.8, 556.9, to all be valid reasons for high risk screenings even if the patient is not having a flare.
