Wiki complete vs incomplete colonoscopy

In the past we have coded a colonoscopy as complete if the dr. goes beyond the splenic flexure. However, we are now being told that we must use a modifier if the dr. does not reach the cecum. CPT definition of 45378 says PROXIMAL to splenic flexure so I am confused. Has anyone else run into this? How are you coding?
