Much to the disappointment of the medical associations across the country, CMS decided to not cover the CCCC codes in CPT, and as such, there are no corresponding HCPCS codes to use.
As far as whether or not commercial payers will cover, I would recommend you contact your payers independently to ask...but usually if CMS says "no" to new codes, the commercial payers follow suit.
In a December (I believe) letter from several associations (AAFP, etc.) these physician groups commented to Marilyn Tavenner to urge CMS to cover those codes for 2014. Stay tuned!
