My complex closure is 5mm on the left abdomen and 10mm on the right abdomen. Do I add the mm together? Or do I bill 2 code based on size?
 
Thank you! My doctor puts in the notes the size of the lesion and the size of the complex closure. Which number do I go with? For example: lesion 20mm complex closure 45mm. This would be two different codes depending on which size I use.
 
In your first example it would be 15 mm, unless your provider added in the margins (for example - 15mm + 2 mm margins would total 19mm. Margins are 2 sides). If margins aren't listed, then I would query the provider.
 
Just realize the lesions are coded individually based on size of excision (size of lesion plus margins).
Repairs (when separately billable) in the same anatomical area of the same complexity are added together. The size of the repair(s) should be documented.
