Wiki Compliance violation

I have an incident that violates our compliance program. What is the best way to write this up? Is there a form of some kind? This is the first time a violation has occurred so it needs to be addressed so that the compliance committee is aware of what has happened to take action and try to prevent anything like this from happening again. Thanks so much for any input you may have.

No. The compliance program was just started a few years ago and to my knowledge this is the first time for a violation. In the next compliance meeting I will be sure to address how to report an incident if it happens again.
