What codes should be used when two "Chemo" drugs IVPB (ie. Pertuzumab & Trastuzumab) are infused concurrently for 30 minutes?

When would it be appropriate to use the 96549 Unlisted Chemo procedure code?

If two chemo drugs are infusing through the same IV line you can only code one infusion procedure code, but can code both drugs. Concurrent means the drugs are infusing into 2 separate IV sites, not piggybacked.

I was taught not to use 96549 because it's usually flagged and rejected by carriers.
