Is the following correct:

Hycamtin - 10:15 am - 10:20 am 96409

Zofran - 10:25 am - 11:25 am 96367

Decadron - 10:25 am - 11:25 am 96368

If not correct what should it be

Zofran and decadron are usually given/documented in IVPB (piggy-back) fashion and thus eliminates the 96368 code. Typically, the scenario to use the concurrent infusion is when two different types of drugs are used at the same time, chemo - therapeutic.
