Concurrent infusion timing

I have questions regarding the coding of concurrent Chemo/Therapeutic admin. If a Chemo (AVASTIN) admin is started running for 30 mins in total (11:15 - 11:45) & 10 mins into that infusion a Therapeutic (LEUCOVORIN) is started running for 2 hrs (11:25 - 13:25) How would the administration be coded for these?
 
Another question...

What is the proper coding for a 10 min Chemo push administered an hour into a 2 hour Therapeutic infusion? (So one CIVP administered during a 2hr Therapeutic Infusion)
