We have a case where Oxaliplatin ran from 10:35 to 12:35, Leucovorin from 10:35 to 12:35 then Magnesium 9:51 to 11:51 then 11:51 to 12:51. I know
that we would bill 96413, 96415 and 96368 for Oxaliplatin and Leucovorin. Would we bill 96367 and 96366 x2 for Magnesium or let it count under the 96368
as well as the Leucovorin? We are seeing a lot of these, and they are so confusing! Just want to be sure not to over bill or under bill
