concurrent infusions

I have 3 therapeutic drugs given at the same time do I code only one concurrent infusion 96368 or can I code one concurrent infusion and one sequential 96368 & 96367.

Is the concurrent infusion code for "2 or more" drugs? or only 2 drugs given simultaneously?

Thanks for your help, I am new to chemo coding.
 
If all are given at the same time, not one after the other, then you must code 96365, 96368 (only useable once per billing). Add on 96366 if at least 1 hour, 31 minutes, then again for 2 hours, 31 min, etc.
