Wiki Concussion Coding for Outpatient Rehabilitation Facility Question

I have a patient that is being referred for outpatient rehabilitation due to a concussion sustained over two years ago. The patient's pcp has just referred the patient with the diagnosis of "concussion with no loss of consciousness". I am using Icd.10: S06.0X0D, (still in the healing, recovery phase, right?) there is no mention of post concussive symdrome. Patient presents with multiple complaints such as: memory loss, poor concentration and attention span, moody, emotional, anxious, insomnia, headaches, dizziness, falling, imbalance, depression. Am I correct is using S06.0X0D? Should I be coding all of the signs and symptoms? I am thinking that the signs and symptoms should not be coded at all since "concussion" encompasses all of these? Can someone please let me know if I'm on the right track? Thank you!
