Hi everyone. I have a question regarding the Conditions of Participation that an inpatient facility is required to have to be able to offer services. There seems to be some disagreement about what chart documentation is required to be able to bill for services at a psychiatric hospital, in Washington state, and it was my understanding that the following documents must be in a chart:
  1. Signed consents -notice of privacy practices, consent for assessment, consent for admission, and rights and responsibilities (all signed or documented that patient is unable to sign).
  2. Psychiatric evaluation to be completed within 24 hours of admission,
  3. H&P to be completed within 24 hours of admission,
  4. Discharge summary.
  5. It's also been suggested that a PE is not required to bill for services only a DC summary.
Can someone please help with information. I have looked through the WACs and RCWs trying to find clear information.
