Hello there -

I just came back from maternity and have observed a changed in the reports I am getting to assign codes for. The providers used to spell out what the medical conditions are but now I am only getting the ICD-10-CM codes written/ printed on them.

I had not been successful in finding the official guidelines/ verbiage that code assignments should be based off conditions spelled out/ specified and not just by codes chosen by providers from the EHR system. Anyone can point me in the right direction so I have supporting materials when I broach this issue with my bosses?
