We have physician's who complete the Conner's Questionnaire for ADD/ADHD. One of our physicians believes there is a code that will allow them to be reimbursed for completing this form. We usually just charge an E/M code and tie it in with the visit. Other than the prolonged physician service code does anyone else charge for this service? It is completed after the patient has left and is forwarded to the patient's teacher. Help! :D
 
We bill a 96110 for this. Dr sends form home or to teacher and when it is returned, dr reviews it and writes a report summarizing it. We won't bill it until after dr gets it back, incase they don't return the form as planned.
