Can someone tell me for what procedures done in the clinic does a consent form need to be signed by the patient before treatment. For example: lesion removal. Is there a general rule?

In our clinics, we get a consent form for any "procedure", including injections, EMG and NCS, derm procedures, etc.

Yes, you'd want a consent for any procedure. We require consents for excision or biopsy of lesions performed in our office.

Consent form every single time
Just curious, if the treatment is a series , do we always obtain consent form per visit? Example Botox Injection for same area every 3 months.

I would obtain consent for every procedure, each time it is done. CYA.
