Hi - I have a physician who is attempting to bill 99222 based on this documentation. I don't agree ...looking for some support. Thank you

Consultation Report

She has obviously malignant masses in bilateral lungs, but she also has a 3cm L supraclavicular mass that can easily be biopsied, and she also has symptomatic biliary disease that will probably turn out to be metastatic lung CA, but this has yet to be established. I recommend we biopsy the supraclav mass tomorrow and get GI involved to discuss management of her obstructive jaundice. A lung biopsy doesn't add much to the database at this point but can always be an option in the future depending on how events unfold.

I have one additional question - if this was a consult (Medicare patient) so the 9922X would be appropriate. Is it sufficient to put 'reviewed case' implying that the HPI, Exam, etc. performed by the hospitalist was reviewed? Or does that need to be listed separately in their note as well?
