it is my understanding that Medicare no longer pays on consults (example code 99253) how do you now code initial consultations for inhospital consults or any consultation

There is CMS transmission on this and many many post on this forum on this subject which I encourage you to look at. An inpatient consult will use the initial hospital (or initial nursing facility) visit codes with no modifier. The office or outpatient consults will be billed with regular new or establish patient visit levels.
