Does it violate our contract with an insurance company if we have the patient sign an ABN to not bill their insurance and pay cash for specific services (ex: testoterone injection)

No, patients now have a legal right to request portions of their health record stay private even from their insurance. You will need a way to notate this in the chart. I believe this was part of the HIPAA changes a few years ago.
