Control of oropharyngeal Bleed Denied

A 3-year-old patient returned to the operating room (OR) on the same day as a tonsillectomy due to an oropharyngeal bleed. CPT code 42962 was performed, and the claim was submitted to GEHA with modifier 78. The insurance company denied the claim using denial code CO 16 (Invalid days/units of service). How do I fix this claim?
