I received a claim that was denied. After reviewing the information and the op report, I see that the claim was incorrectly coded by someone else and it also missing a code. Can I make the appropriate changes to the cpt codes by changing the code and also by adding a code? part of the claim would be coded completely different than when it was first submitted.

Follow whatever the payers process is for submitting corrected claims.
