Wiki Coumadin Management after Ortho Surgery

Hi all!

My question involves management of routine anticoagulants after surgeries such as knee or hip replacement. Since our ortho surgeons routinely prescribe prophylactic anticoagulation for all of these patients, should the nurse visits for titration be billed under the surgeon with a modifier 79 or should it be considered part of the global surgery since it is the routine rather than the exception?

I apreciate any and all opinions.
