Has anyone had any luck billing the CPT 99072? I have a payer denying stating "SERVICE OR SERVICE/MODIFIER COMBO NOT FOUND ON FEE SCHEDULE". I am not using a modifier, does anyone have any suggestions what would modifier you are using or what may be sufficient to use?

There have been a couple of other threads with this same question/topic.
Summary:
Medicare never pays under any circumstances.
Commercial carriers will never/rarely ever pay. I'm not aware of any majors that do. Some small ones do - < 1% from what I have seen.
Per the other thread, WC and auto do, but I have no personal experience with either.
