Figured I would start this thread as I'm sure I'll have questions about the exam content.

First question is, the two pictures below are from the practice exam offered by the AAPC online and the practice exam offered by the AAPC in the paper study guide. (I bought both).

Am I interpreting this wrong ??

Sure seems to me the "Quarterly" answer is correct.

This is a bit disconcerting since I am studying for my exam as well.

I have a question regarding HIPAA and PHI disclosure.

You don't need a patients or the patients rep's authorization to disclose PHI if,

"Treatment, Payment, healthcare operations or to another entity for treatment purposes, for quality or competency assurance activities or fraud and abuse detection and compliance activities if both entities have or had a relationship with the patient and the PHI relates to the relationship"

What about indirect entities? Say Medicare contracts with a vendor to gather information needed to reassess the NCD or LCD. Can that vendor ( third party, no direct patient relationship) be disclosed PHI ?
