Can anyone advise me on what type of closures are included in cpt 19120? My provider well documented a difficult CLC, op notes were sent with claim and closures was denied as inclusive to 19120. Thoughts?
 
All closures are inclusive to this procedure. If any part of the procedure is well documented as difficult and the reason why you should use the 22 modifier.
 
I guess that could be an accurate summation. You use the 22 when the procedure is documented as being more involved that what is normal for that procedure and no other procedure code describes what was performed. The reason for why the procedure is more involved needs to be clearly evident in the body of the note.
