Can someone help with the following note. Does the levels get counted? or is this just cpt 20550 with 77003/26
Procedure Note: After obtaining written consent, pre-procedure blood pressure and pulse were obtained and recorded in the chart for review. The patient was placed in the prone position. The lumbar spine was prepped with betadine times three and draped in the usual sterile fashion. The skin over the L4-5 interspace was infiltrated with 0.5% Lidocaine for local anesthesia. With fluoroscopic guidance, a 25G needle was directed between the L4 and L5 spinous processes, middepth. Repeated at L5-S1. Following negative aspiration, a total mixture of Bupivacaine 0.25% 1 cc's with Depomedrol 80 mg was injected with minimal pressure. There was no evidence of CSF, paresthesia or heme. Following the procedure the patient's vital signs were stable. The patient was discharged home in good condition after being given discharge instructions.
