Good morning,

Neurosurgery is billing CPT 62141 which is correct however, he doesnt specify in his note the size. my question is since this code specifies greater than 5cm does the size have to be documented on his note. please help he doesn't think it matters.

Yes it matters. You cannot know if the code is correct or not if the size was not documented in the record. ‘Greater than 5 cm’ is a part of the code description and must be supported by documentation.
