Modifier KX for cpt code 64494? Medicare denied the code as bundled would a KX mod be allowed?

Hi there, I'm not sure why it was denied as bundled. At any rate, you should only report KX with a diagnostic block and if the level 1 didn't have the modifier that could raise questions.
