CPT 64614 Rejections

I am in Alabama, and my Pediatric Rehab physician regularly administers botox injections(64614). Alabama Medicaid has started rejecting the injections stating that a modifier is required to file these charges. Is anyone aware of a change in the requirements for filing this charge. If so, what modifier is required. Thanks for your help!
 
I would first check with your Alabama Medicaid web site to see if they use certain or specific modifiers that wouldn't be in your CPT book.
For example in California, we have modifiers specific to women's health such as ZL and ZS. These are Medi-Cal specific and wouldn't be used in other insurances nor outside of California.

If you do find the information keep a copy on hand and share the information. This will help reduce any delays for processing claims and compromise reimbursement.
