New to Podiatry, having issues with receiving payments for 64640 billed multiple times even though MUE states 5. Any info would be very much appreciated. Here's one example. What would be the proper way to code it? Thanks!

DIAGNOSIS:
Right foot chronic plantar fasciitis and neuritis.

PROCEDURE PERFORMED:
1. Rt foot radiofrequency nerve ablation of the inferior calcaneal nerve
2. Rt foot radiofrequency nerve ablation of the periosteal branch of the inferior calcaneal nerve.
3. Rt foot radiofrequency nerve ablation of the medial calcaneal nerve
4. Rt foot platelet rich plasma autologous graft implantation, plantar fasciitis.
5. Lt foot radiofrequency nerve ablation of the inferior calcaneal nerve
6. Lt foot radiofrequency nerve ablation of the periosteal branch of the inferior calcaneal nerve.
7. Lt foot radiofrequency nerve ablation of the medial calcaneal nerve
6. Lt foot platelet rich plasma autologous graft implantation, plantar fasciitis.
