Hello! I work claims denials in an ophthamology office. Our doctor bills for lid excisions using CPT 67840 with modifiers 50, LT/RT if both eyes, and E1/E2/E3/E4 depending on location. Insurances continue to deny, stating a missing modifier. What am I missing? Thanks for any help you can provide...

Example of one case: 67840-50, LT, E2 Removal of pappiloma left lower lid
67840-50, RT,E3 Removal of cysts (2) right upper lid
