77334 is generally performed prior to the start of radiation therapy. The patient will not be in the office.

Do the patient pay a co-pay for this code when there is no physical visit?

There is no way to answer that question with a yes or a no. Copays are subject to the rules of the individual patient's plan. Copays may be the patient's responsibility for every service provided (highly unlikely), or attached to an office visit E&M code, or as a percentage of the allowed amount. If the patient is asking, they should contact their insurance company at the number on their member card. If the doctor's office is asking, they can either call the plan or submit the claim and see what happens.
