Hi All,

We billed CPT 85018 (QW) to Medicare. But we are getting a denial stating " Submission billing error" Can anyone help me with this?

You will use the QW mod if the concerned practice has a CLIA Certificate of Waiver and the CLIA# should also be submitted with the claim.

I completely agree with Parth you need to submit CLIA Certificate of Waiver and the CLIA# should also be submitted with the claim.
